You will be asked to confirm that you have read and will comply with them. \n\n\n\nFood Vendors or Vendors Selling Food\, please review the City of Dover Health Permit Information\, Letter to AHD vendors from the Health Inspector\, and complete all necessary City of Dover Health Permit Forms. You will need a "Class V" health license. Please send a copy of your approved permit to The Greater Dover Chamber of Commerce at 550 Central Ave\, Dover NH 03820. \n\n\n\nAll vendors must submit a valid Certificate of Insurance and name The Greater Dover Chamber of Commerce as an Additional Insured. No exceptions! Certificates must show a liability limit of $1\,000\,000 per occurrence. Please make sure we are able to match the Insured Name with the name on the vendor application.
